OPC Expense Voucher Form and Instructions

Instructions:

Fill out the form which you can access via one of the links below. The form asks for an account number assigned to the particular Session team/committee and project. Click here for the list of account numbers. Submit the completed form to the chairperson (an Elder) of the Session team/committee from which you are requesting funds. Each Elder has a box in the communications room and you may want to call that Elder if getting the voucher approved is urgent. Once the voucher is authorized, it will be submitted to our office manager to have a check written. Checks are normally written on Tuesday mornings. Vouchers need to be signed and submitted by 9 AM on Tuesday morning in order to get processed that day.
